Health Insurance

Health insurance benefits are effective on the date of your hire and are available to you and your lawful spouse, domestic partner, and/or eligible dependents. You may choose participation in either of the HMO plans or PPO plan each calendar year depending on where you are located. Employee contributions towards the cost of coverage may be deducted.

Medical & Prescription Drug

HMO (Access + HMO Plan 10) Blue Shield (California Only)
Services available when they are provided or authorized by your primary care physician in your participating Medical Group/IPA, although you may self-refer to a specialist in your participating Medical Group/IPA with a $30 co-payment. For each type of service provided there are applicable co-payments with calendar-year copayment maximums of $1,500 (per individual) and $3,000 (per family). There are no calendar-year deductibles.

PPO (SHIELD Spectrum PPO 250 90/70 Premier) (California Only)
Allows the freedom to choose your own doctors and hospitals for all of your healthcare needs. Does not require authorization from a primary care physician before seeing a specialist. Office visit co-pay is $20 for Physician and Specialist office visits. There is a $500 calendar-year deductible per individual, $1,000 per family, after which services are covered at 90% coinsurance. Calendar-year co-payment maximums are $1,000 per individual, $2,000 per family. Office visits are not subject to the Calendar-Year Deductible.

Prescription Drug Coverage (CA Blue Shield)
California Blue Shield HMO and PPO plans include prescription drug coverage with a $10 co-payment for generic drugs, a $30 co-payment for formulary or name brand prescriptions or a $50 co-payment for non-formulary drugs at participating network pharmacies for a 30-day supply. Mail-order services that provide a 90-day supply are also available with a $20 co-payment for generic drugs, a $60 co-payment for formulary or name brand prescriptions or a $100 co-payment for non-formulary drugs.

HMO Kaiser (California Only) Traditional Plan
Services available when they are provided or authorized by your primary care physician. There is no calendar-year deductible. Calendar-year co-payment maximums are $1,500 per individual, $3,000 per family. Premiums can be deducted on a pretax basis.

Kaiser prescription drug coverage from plan pharmacies or mail order services is $10 for generic drugs, and $15 for brand name drugs. Both pharmacy and mail order provide a 100-day supply.

PPO (Blue Shield - Highmark - Outside of California)
Allows the freedom to choose your own doctors and hospitals for all of your healthcare needs. Does not require authorization from a primary care physician before seeing a specialist. Office visit co-pay is $10 for primary care and specialists. After a $250 calendar-year deductible per individual, $500 per family is met, services are covered at 90% coinsurance. Calendar-year co-payment maximums are $1,000 per individual, $2,000 per family.

Prescription Drug Coverage (Highmark)
Highmark PPO plans include prescription drug coverage with a $10 co-payment for generic drugs, a $20 co-payment for formulary or name brand prescriptions or a $30 co-payment for non formulary drugs at participating network pharmacies for a 30-day supply. This plan also offers mail order services with a $20 co-payment for generic drugs, a $30 co-payment for formulary or name brand prescriptions or a $60 co-payment for formulary or name brand prescriptions.

Vision (automatically included in medical coverage)

(NVA for PMSLIC and VSP for all other office locations)

Annual eye exam covered at 100% with $20 deductible; allotment for frames every 12 months and lenses/contacts every 12 months. Computer VisionCare (CVC) benefits also available providing for additional examination, special prescription lenses and vision therapy. Laser vision surgery benefits are available and include a free consultation and evaluation plus access to the procedure at a reduced cost.

Dental Insurance (Delta Dental)

Includes 100% coverage for in-network diagnostic and preventive services, 100% coverage for general services and 60% coverage for major services after $25 ($75 per family) annual deductible. Your annual benefit is $2,000. Orthodontic coverage is available with a $1,500 lifetime maximum (adults and children). Network of dental providers is available online.

Flexible Spending Plans (Health Care & Dependent Care) Administered by Benefit Resource

Enrolling in an FSA allows you to make tax-free salary contributions to pay for eligible medical and dependent care expenses that are not covered or reimbursed by any other source. FSAs increase your take-home pay by reducing taxable income, making these out of pocket expenses more affordable! Employees can set aside pre-tax dollars per plan year for reimbursement of eligible medical expenses up to a maximum of $4,000 per calendar year. Using your Beniversal card will allow you to access Medical FSA funds to pay for eligible medical expenses at qualified merchants. The Beniversal Prepaid MasterCard works much like a credit card, but uses funds from your Medical Flexible Spending Account (FSA) to pay for eligible medical expenses at the point-of-sale.

In addition, employees can also set aside pre-tax dollars per plan year for reimbursement of eligible dependent care services up to a maximum of $5,000 per calendar year.

Life Insurance

Life insurance benefits provide basic protection for you and your family. You are eligible if you are an active, full time employee working at least 21 hours per week.

Group Term Life
You are automatically enrolled in coverage equal to 2 times your annual base salary for Group Term Life up to a maximum of $1,000,000. Additional coverage for Accidental Death and Dismemberment (AD&D) of 2 times your annual base salary up to a maximum of $850,000 is also provided. Group Term Life is a 100% company paid benefit.

Group Universal Life (GUL)
This supplemental voluntary life insurance coverage is offered on an employee-pay-all basis you may purchase coverage in a multiple of your salary, from 1 time to up to 5 times annual salary. Life insurance is also available for your spouse and dependent children. You may also choose to save money in a Cash Value account and earn tax-deferred interest. Coverage is portable at time of termination.

Personal Voluntary Accident (PVA)
Offered on an employee-elected basis, provides optional accident insurance with single or family coverage available from $25,000 to $250,000.

Business Travel Accident Insurance
If accidental death or dismemberment happens on company business travel, coverage is 4 times your annual salary (minimum benefit $50,000, maximum benefit $250,000). Coverage is automatic for those eligible and 100% paid for by the Company.

Retirement Benefits

401(k) Plan (Administered by the Principal Financial Group)
Employees are able to defer on a pre-tax basis up to 100% of eligible pay. Participating in the 401(k) plan provides employees with an added benefit by reducing their income for Federal income tax purposes since deferrals are made on a pre-tax basis, which, in turn, reduces employee's tax obligation.

NORCAL will match on a biweekly basis dollar for dollar up to 5% of eligible compensation that employees defer. Eligibility for the match requires one year of service and the match becomes immediately 100% vested. Additionally, after one year of service, employees are eligible to receive an annual discretionary company contribution, calculated and paid at the end of each calendar year. This vesting schedule for this contribution is 20% at 2 years, 50% at 3 years, 60% at 4 years, 80% at 5 years and 100% vested at 6 years of service.

You may check your investments daily via the plan administrator's website and may be eligible to take out loans per the 401(k) plan provisions.

Pension Plan
NORCAL's pension plan provides retirement income to employees calculated using a formula that uses years of service and eligible earnings. Funding for this benefit is the responsibility of NORCAL.

The formula for calculating the earned pension benefit is: 1.5% times average monthly salary from the highest 5 service years multiplied by your total years of service (30 maximum). This benefit will be paid to you at the time of retirement based on the annuity option you elect at that time. Employees are 20% vested in the plan after 3 years, 40% after 4 years, 60% after 5 years, and 80% after 6 years and fully vested after 7 years.

Time Off

Paid Time Off (PTO)
PTO accrues at 5.3846 hours per pay period to a total of 20 days accrual for each of the first through five years of service and then increasing after 5, 10, 15, 20 and 25 calendar years. PTO can be used for your scheduled vacation, sick time or other personal needs.

Holidays
NORCAL Observes the following holidays each year:
  • New Year's Day (1 or 2 days)
  • President's Day
  • Memorial Day
  • Independence Day
  • Labor Day
  • Thanksgiving Day
  • Day After Thanksgiving
  • Christmas (2 days)

Disability Benefits

Extended Disability Leave (EDL)
Certified disabilities lasting 11 or more consecutive days are paid at 100% of salary less money paid by a third party. The benefit is paid based on the length of your service (6 weeks of salary for 7 months through 2 calendar years of service, 12 weeks of your salary for 3 or more calendar years of service).

Long Term Disability (LTD)
After 90 days of certified disability or when EDL is exhausted (whichever occurs later), you are elgible to apply for coverage under the Long Term Disability benefit, which pays 66.66% of your salary up to $10,000 per month. If you elect to pay the tax on the LTD premium, any future LTD benefits will be tax-free.

Other Benefits/Programs

Employee Assistance Program
Available to employees working a minimum of 21 hours per week, our Employee Assistance Program offers confidential clinical counseling services for you, your family and/or your legal spouse/domestic partner. The plan provides up to 5 free in-person sessions per household member, per concern, per year. Additional phone support services are available on an unlimited basis and include financial consultations, child care consultations, elder care consultations, legal counseling, and a nurse advice line.

Educational Assistance
Available to employees who work 30+ hours a week. Just because you start working at NORCAL doesn't mean you have to stop learning. Off-the-job development activities that tie in with the company may be reimbursed up to 100%. You may be reimbursed for books and pre-approved courses for which you receive a grade of C or above.

Wellness Reimbursement
After 6 months of service, you may be reimbursed 50% (up to $500 per year) of your annual expenses for health club membership dues, smoking cessation or weight loss programs, etc. (Receipts must be submitted to qualify for reimbursement.)

Workweek and Office Environment
NORCAL is a business casual work environment with a regular work week of 35 hours.

Credit Union
Offered on an employee-elected basis, provides special rates on your checking, saving and investment accounts.

Tax-Free Parking & Transportation Reimbursement Plan (California Only)
Offered on an employee-elected basis, you may put aside pre-tax dollars to pay for certain work-related parking and/or transportation expenses. Only expenses for qualified work-related parking and transportation under Internal Revenue Code (IRC) section 132(f) are eligible for reimbursement under the plans.

MEDICUS BENEFITS


Health Insurance

Blue Cross/Blue Shield of Texas
Medicus Insurance Company provides medical insurance coverage for all full-time employees. For the 2012 plan year, you have a choice of 2 PPO medical plan options for BCBS. BCBS is a nationally recognized carrier with one of the largest networks to utilize. If you enroll, your coverage will become effective the first of the month following 90 days from your date of hire. BCBS also provides discounts through the Davis Vision Plan.

Flexible Spending Plans

Flexible Spending Accounts (FSA) provide you with an important tax advantage that can help you pay health care and dependent care expenses on a pre-tax basis. By anticipating your family's health care costs for the next year and utilizing an FSA, you can actually lower your taxable income.

Health Care FSA
Medicus Insurance Company will contribute $1,500 to each employee's account. Employees may contribute up to $3,500 in addition to the $1,500. Examples of expenses not covered in your plan include:
  • Deductibles and copays
  • Dental services and orthodontia
  • Vision services
  • Prescription drugs
  • Chiropractic services
  • Acupuncture
  • See irs.gov for a comprehensive list of qualified expenses
Dependent Care FSA
This program allows Medicus Insurance Company employees to use pre-tax dollars for dependent care. The maximum amount that may be contributed to the Dependent Care Reimbursement account is $5,000 per year per family. All eligible dependent care expenses must be employment related and include some of the following:
  • Day Camp – primary purpose must be custodial care and not educational in nature.
  • Dependent care expenses that are necessary for you (and your spouse) to work, actively look for work, or attend school full time.
  • Other available additional eligible expenses.
The TASC Card
The TASC card provides a convenient method to pay for eligible healthcare, dependent care, and/or transportation as defined by your FlexSystem plan. MyBenefits is funded by you and your employer through equal pre-tax payroll deduction based on your annual benefit election. The TASC Card is available for the following FlexSystem Accounts (where applicable):
  • FlexSystem Healthcare FSA
  • FlexSystem Dependent Care FSA
  • FlexSystem Transportation Account

Disability Insurance

Medicus Insurance Company provides all active full time employees working at least 30 hours/week with short and long term disability income benefits through American General. In the event you become disabled from a non-work related injury or sickness, disability income benefits are provided as a source of income. You are not eligible to receive short-term disability benefits if you are receiving worker's compensation benefits.

401(k) Plan

The Medicus 401 (k) Profit Sharing Plan is a great way to save for retirement. It is the goal of Medicus Insurance Holdings, Inc. (Medicus) to continue to help you build a more secure financial future through The Principal Financial Group. The company matches 100% the first 3% of an employee's contribution and 50% of the next 2% to a maximum contribution by Medicus of 4% if you contribute 5% or more.

Other Benefits

Employee Assistance Program
Medicus, through the American General Insurance Company website, offers an employee assistance program. The American General Employee Assistance Program (EAP) can be accessed with a user name and password and will provide employees with valuable resources regarding personal and workplace issues. Topics could include work/life transitions, legal and financial assistance, and emotional well being, among others. Further details can be obtained through your Human Resources Representative.

PTO/Holidays
Medicus observes 12 holidays per year and provides employees with a total of 12 PTO days per calendar year. Full time employees will be eligible to receive full pay for holidays upon completion of ninety (90) days of employment. Employees starting prior to July 1 will be granted two personal days for that calendar year.

Excellence
